Copy the actual sentence from the page rather than a softened summary. Record the page URL, section, intended audience, and the decision the claim might influence. Classify it as a feature statement, measured outcome, comparison, testimonial, or editorial opinion. This small distinction changes the type of evidence required. Begin with consequential claims about price, availability, eligibility, or results instead of cataloging every harmless introductory sentence.
Attach support and its boundary
Add the exact supporting source and the relevant passage, plus dates and scope. Write one sentence stating what the evidence does not establish. A limited internal test may support a narrow observation without proving a universal benefit. A customer quote may be authentic without being representative. If no suitable support exists, assign an action: research, narrow the wording, label opinion, or remove the statement. Do not manufacture evidence to complete the ledger.
Assign ownership and review triggers
Give each consequential claim a responsible person or team. Review triggers should reflect the source of change: a product release, pricing revision, new service area, or expired research period. Use the ledger during revision
